Format
| OTR | COTA | |
|---|---|---|
| Items | 180 | 190 |
| Exam time | 4 hours | 4 hours |
| Appointment length | about 4 h 45 min | about 4 h 45 min |
Two item types appear on both forms: three- and four-option single-response multiple choice, and six-option multi-select items, which on the OTR form are presented as scenario sets. Multi-select items take longer, so the effective pace on the single-response items must be faster than the average.
OTR domain weights (content outline, January 2024)
| Domain | Weight |
|---|---|
| 01 Evaluation and Assessment | 23% |
| 02 Analysis, Interpretation, and Planning | 23% |
| 03 Select and Manage Interventions | 38% |
| 04 Competency and Practice Management | 16% |
COTA domain weights
| Domain | Weight |
|---|---|
| 01 Collaborate and Gather Information | 27% |
| 02 Select and Implement Interventions | 55% |
| 03 Uphold Professional Standards and Responsibilities | 18% |
The COTA blueprint is even more concentrated: more than half the exam is implementing interventions under OTR supervision. On both forms, intervention selection and implementation is where the exam is won.

Exam guides
- NBCOT exam cost: the fees the handbook actually names
NBCOT publishes the current exam fee on its Get Certified page rather than in the handbook, so confirm it at application time. The handbook does name the fees that surprise candidates: $115 to reissue an Authorization to Test letter, $35 for a duplicate MyNBCOT account, and $35 for each score transfer or eligibility confirmation notice requested after application payment.
- NBCOT exam application: eligibility, timing and the 90-day window
Applying for the NBCOT exam requires a MyNBCOT account, academic verification from an ACOTE-accredited program, a character review and a background check. Once submitted, the exam application is valid for 90 days, and the Authorization to Test letter that follows has its own expiry — arriving late, no-showing or rescheduling inside 48 hours costs a $115 ATT reissue fee.
